Have you been told you need to change your diet due to a medical problem or to avoid a medical problem?  Let us help you to make the transition in the easiest and simplest way possible.  We will help you to identify what is causing the health or the weight problem.  Once identified we will work with you and  help you to set a plan that meets your needs and helps you to  reach your health goals.  W e work as partners with you to help you succeed.

HOW TO START

Call our office.  Joan, our administrative assistant, will take your information.  This includes your full name, address, phone numbers, insurance, reason for the referral and your physician’s name.  We ask that you have your insurance identification number available at the time of your call.  We will confirm your insurance coverage with your insurance carrier.  We will always inform you if your insurance company does not cover medical nutrition therapy or nutrition counseling.

INSURANCE COVERAGE

We are currently providers for Aetna, Anthem Blue Cross, Cigna, ConnecticutCare and Oxford.  We are also Medicare providers.  Be aware, that at this time, Medicare only covers diabetes and kidney disease.

SELF PAYMENT

Clients can and do pay when their insurance plan does not have a nutrition benefit.  Please check with us on the current rates if you plan to self-pay.  However, be aware that insurance coverage is increasing for all diagnoses.  If your plan does not have a nutrition benefit today, it may include the benefit next year.

SCHEDULING THE  APPOINTMENT 

We will schedule your appointment in the office and at the time most convenient for you.

PREPARING FOR YOUR APPOINTMENT

Before your visit, you will be asked to complete the client information form and the food frequency form which can be downloaded from this site under “Client Forms“.  There is a different form for children & teens than the one for adults.

It is also helpful, but not necessary, if you can keep food diaries for 2-3 days prior to your visit.  Keep a day on the weekend and 1-2 days during the week.

If you have the diagnosis of diabetes and test your blood sugar  daily or even ocassionally, bring your records or your glucose meter to the appointment.

MEDICATIONS/SUPPLEMENTS

List both prescription and over-the-counter medications/supplements on the client information form or bring the information on another sheet.  If you take any supplements other than multivitamins or calcium, such as a protein supplement, please bring the bottle or package to your appointment.

CONFIRMING YOUR APPOINTMENT

A day or two before your appointment you will receive a call to remind you of your appointment.

CANCELLATION

We understand that things come up that are out of your control and you may need to cancel.  You are responsible for any appointments not cancelled 24 hours in advance.  If you do not cancel in this time frame we will bill you for the visit.  We cannot bill your insurance company for an appointment you did not attend.
